Public directory data

Directory records are assembled from public provider and organization sources. A public record may include organization names, business addresses, business phone numbers, NPI identifiers, and public authorized-official fields.

Information you submit

Correction, claim, and verification forms collect your name, email address, practice name, organization NPI, and the message or evidence you choose to provide. Do not submit patient information, medical records, credentials, payment-card data, or other sensitive clinical information.

Human verification and infrastructure

Cloudflare provides hosting and human-verification services and may process network and device information under its own terms. DoctorOwned uses the request IP to validate the human-verification token but does not intentionally store the raw IP in the application record.

Use and retention

Submitted information is used to investigate the request, communicate with the submitter, prevent abuse, and preserve an evidence trail. DoctorOwned has not yet published an automatic deletion schedule. A submitter may request review or deletion by using the contact below, subject to record-integrity and legal needs.
